TO COMPLETE THE BOOKING PROCESS

  1. PRINT (or email) the Parental Consent / Medical Form to send home.
  2. SEND form to parents to be returned ASAP to finalize your head count.
  3. CONFIRM your participant numbers by email groups@hornelake.com
  4. BRING all of the signed Parental Consent / Medical Forms with you to the Cave Park.
GROUP LEADER CHECKLIST

NOTE:

  • Group Leader must confirm final numbers by email to education@hornelake.com
  • Group Leader must arrive with a signed Parent Consent / Medical form for every participant under 19 yrs of age.
  • ALL participants age 19 or older must sign a colour copy of the Liability Waiver (includes Teachers, Group Leaders, EA’s, Parents, etc.) These can be signed in advance or at the cave park the day of the tour.
